All citations are created in the spirit of\u00a0<em><a href=\"http:\/\/amzn.to\/1H8sTPT\">Evidence Explained<\/a>.\u00a0<\/em>The first post in our &#8220;Citing Social Security Items&#8221; will focus on the SS-5 form.<\/p>\n<p>It seems fairly straightforward to cite an SS-5 form such as the one for Anna Margaret Lake shown in this post. The &#8220;first reference note&#8221; for this form should be:<\/p>\n<blockquote><p>Anna Margaret Lake, Social Security Number 340-38-7070, 12 February 1963, Application for Social Security And Tax Account Number (Form SS-5 [revised August 1962]), Social Security\u00a0Administration, Baltimore, Maryland.<\/p><\/blockquote>\n<p>That&#8217;s pretty much how the form is cited in\u00a0<em><a href=\"http:\/\/amzn.to\/1H8sTPT\">Evidence Explained<\/a>&#8212;<\/em>I did use the form title as on the actual form. It&#8217;s possible that the form has cha<img loading=\"lazy\" decoding=\"async\" class=\"alignright wp-image-616 size-medium\" src=\"https:\/\/rootdig.genealogytipoftheday.com\/wp-content\/uploads\/2015\/07\/apgar-lake-300x167.jpg\" alt=\"apgar-lake\" width=\"300\" height=\"167\" srcset=\"https:\/\/rootdig.genealogytipoftheday.com\/wp-content\/uploads\/2015\/07\/apgar-lake-300x167.jpg 300w, https:\/\/rootdig.genealogytipoftheday.com\/wp-content\/uploads\/2015\/07\/apgar-lake-1024x569.jpg 1024w, https:\/\/rootdig.genealogytipoftheday.com\/wp-content\/uploads\/2015\/07\/apgar-lake.jpg 1132w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/>nged slightly over time. My personal style preference going forward is to abbreviate minimally so I&#8217;ve written out &#8220;Social Security&#8221; instead of abbreviating.
